Muscle spasms, what, why, and how.

Most causes of muscle spasms are not what you think…

When a muscle becomes painful, tight or difficult to move, it is often described as being “in spasm.” Many people immediately assume that they are dehydrated or deficient in magnesium or another electrolyte.

Electrolyte disturbances can cause muscle cramps in certain circumstances. However, the tight, painful muscles commonly experienced in the neck, shoulders, and back are usually better explained by changes in pain, muscle sensitivity, and nervous-system control.

Understanding the difference helps us choose a more appropriate treatment.

What is a muscle spasm?

“Muscle spasm” is a generic term or a symptom. People use it to describe several different experiences:

• A painful muscle that feels like it's locked, like a nerve is being pinched or tight and hard
• Sudden cramping during exercise or at night
• Muscles tightening around an injured or irritated area
• An involuntary twitch or movement
• Muscle overactivity caused by a neurological condition

These experiences can feel similar, but their underlying mechanisms are different.

Protective muscle guarding

Most of the neck and back “spasms” seen in a musculoskeletal practice are better described as protective muscle guarding.

When an area becomes painful, irritated or feels vulnerable, the nervous system may change how the surrounding muscles are recruited. Some muscles increase their activity, movement becomes stiffer, and the body may avoid positions that are perceived as “going to be pain-producing.” IE forward bending to put your shoes on when your back is in a “Spasm”.

This is not necessarily a muscle malfunction. It is often the nervous system attempting to protect the area.

The muscle may feel:

• Tight or tender
• Difficult to stretch
• Painful when moving
• Harder than the same muscle on the opposite side
• Temporarily “locked”

This does not mean that the muscle is permanently contracted, shortened or physically stuck.

How is a true muscle cramp different?

A true muscle cramp is a sudden, painful and involuntary contraction. A calf cramp during running or while sleeping is a typical example.

During a cramp, motor nerves repeatedly stimulate the muscle fibres. Exercise-related cramps are thought to involve altered neuromuscular control, particularly when a muscle is fatigued and working in a shortened position.

This differs from the more sustained feeling of stiffness or guarding commonly associated with neck and back pain.

Are muscle spasms caused exclusively by low electrolytes?

Not automatically. Magnesium, sodium, potassium and calcium are essential for normal nerve and muscle function. A clinically significant deficiency can contribute to cramping or abnormal muscle activity.

However, a tight trapezius muscle or acute lower-back “spasm” is not, by itself, evidence of a magnesium deficiency.

Electrolytes deserve more consideration when symptoms occur alongside factors such as:

• Prolonged or very heavy sweating
• Vomiting or diarrhoea
• Poor nutritional intake
• Certain medications, including some diuretics
• Kidney or gastrointestinal disease
• Widespread or recurrent cramps
• A deficiency confirmed through an appropriate medical assessment

Research into exercise-associated cramping does not support a simple explanation in which every cramp is caused by dehydration or electrolyte loss. Fatigue, previous cramping and altered nerve control appear to be important factors.

Taking magnesium without evidence of a deficiency may therefore do little for a painful muscle that is responding protectively to joint irritation, increased training load or pain.

How can Chiropractic adjustments help?

A chiropractic adjustment provides a quick mechanical stimulus to the joints, muscles and sensory receptors around the treated area.

This sensory input may temporarily influence how the nervous system processes pain and movement. Some patients experience:

• Reduced pain sensitivity
• Less perceived stiffness
• Improved range of motion
• Greater confidence when moving
• Reduced protective muscle activity

An adjustment does not replace magnesium, force a muscle to relax or put a displaced vertebra back into position. Its effects are better understood as a short-term change in sensory processing, pain and movement.

This improvement can create an opportunity to move more comfortably and begin the exercises needed to restore confidence and physical capacity.

How can Dry needling help?

Dry needling involves inserting a thin, sterile needle into selected muscle tissue or sensitive points.

The needle creates a strong but controlled sensory stimulus. This may temporarily change local and nervous-system pain processing, increase pressure-pain tolerance and reduce the feeling of muscular tightness.

Some people experience a brief local twitch during treatment. This is a spinal reflex, not proof that a knot has been broken up or that toxins have been released. A twitch is also not required for treatment to be helpful.

Dry needling may help by:

• Reducing local pain sensitivity
• Changing sensory input from the treated muscle
• Improving movement in the short term
• Making exercise or rehabilitation more comfortable

The response varies between individuals, and dry needling is not necessary for every case.

Why hands-on treatment is only part of the solution:

Chiropractic adjustments and dry needling may reduce symptoms and make movement easier, but they do not always address the reason the problem developed.

Longer-term management may also involve:

• Gradually returning to normal movement
• Strengthening the affected area
• Adjusting training or workplace load
• Improving sleep and recovery
• Taking regular movement breaks
• Addressing fear or uncertainty around painful movement
• Investigating persistent or unexplained symptoms

The aim is not simply to “release” a muscle. It is to reduce sensitivity, restore comfortable movement and help the area tolerate normal activity again.

When should recurring spasms be investigated?

Seek an appropriate medical assessment if cramps or involuntary contractions are frequent, widespread, worsening or accompanied by:

• Significant muscle weakness
• Loss of sensation or persistent pins and needles
• Problems with balance or coordination
• Fever or systemic illness
• New medication use
• Persistent vomiting or diarrhoea
• Changes in bladder or bowel control
• Unexplained muscle wasting

These features may suggest that the problem is not ordinary musculoskeletal guarding.

The main takeaway

A painful, tight muscle is not automatically deficient in electrolytes.

In many cases, particularly with neck and back pain, the nervous system changes muscle activity in response to pain, irritation, fatigue or perceived threat. Chiropractic adjustments and dry needling may help reduce pain and muscle sensitivity in the short term, while movement, exercise and load management support longer-term recovery.
